Departmental Checklist for New Employees

In an effort to assist you with coordinating all your needs when setting up access for new employees, we have developed this page as a central starting point.

Listed are links to various departments' information and / or on-line forms which should be filled out for employees, whether they are a new hire or rehire. 

Institutional Requirements:

1)  Submit FAPT to appoint new employee.

2)  Send Selection Packet to HR Recruitment.

3)  Provide New Employee Packet to new employee. 

All new employees are required to visit the Initialization Office with their completed employee packet prior to their first day of work.  Failure to do so may result in a delayed first paycheck.

4)  New Employee Orientation. Your employee will be scheduled for Orientation during their visit with the Initialization Office.  Orientations are held every Monday.   Please be aware that your employees will need to come back to the Benefits Office to sign up for their specific benefits.

5)  ID Badge and Keys.   Your employee will receive their ID Badge during their visit with the Initialization Office.  For Smart Card access, your key control officer will need to contact Campus Police.  This can only occur after the employee has received their ID Badge. 

6)  Employee Health Assessments / TB Tests. All employees are required to have a base line TB Skin Test.  Your employee needs to contact the Employee Health Clinic at (409) 747-9172. The office is located at the Primary Care Pavilion, Suite 124, Entrance B, Harborside Drive.

7)  Required Training.  Your employee will receive the following training during Orientation:

Universal Precautions   
Hazardous Communications   
General Compliance    
HIPPA    
EEO / Diversity / Sexual Harassment

We have provided your employee with the link to the Compliance Training page which has additional on-line training courses that they will need to complete.

Departmental Requirements:

1)  Information Services:

    Long Distance Calling Card
    Cellular Telephone Lease Agreement
    Pager Services

2)  Online access requests for:

EMAIL
HR CICS Transaction (Mainframe)
Smart Card Readers
Digital Certificates

3)  System Training - New Employees that will have financial responsibilities (i.e., requisition, purchase order, and travel processing) must complete EDS (Electronic Document System) training.  HRMS training is also scheduled here.

4)  Departmental Orientation should include:

Performance Management Program / Smart Goals
Emergency Management   
JCAHO Compliance    
Job Specific Required Training    
Computer / Telephone Access 
Departmental Policies and Procedures
