Job Summary:
To provide skilled and specialized clerical assistance in abstracting complex patient-related data from hospital medical records to establish correct charges for professional services rendered for third-party payments.
Major Duties / Critical Tasks:
Tracer Duties:

Retrieves medical records from anywhere within UTMB hospitals / clinics.

Updates computer systems and maintains accurate statistical information reflecting the movement of medical records.
Abstractor Duties:

Abstracts relevant information such as professional services rendered (admissions, daily care, tests, procedures, consultation, discharges) and diagnoses provided.

Verifies that physician documentation is entered on a daily basis and contains the correct verbiage and quality for professional fee billing purposes and proper assignment of levels of service.

Maintains a daily log of the number of patients admitted, discharged and transferred in or out, submitted for review and billing, and on hand at the end of each day.

Notes attending schedules for each ward to establish correct documentation as to which physicians are overseeing the care of each patient to ensure accurate preparation for billing.

Provides daily reporting to immediate supervisor regarding those medical records not considered to be properly documented.

Adheres to internal controls established for department.

Performs related duties as required.
Required Education / Experience:
MRT program graduate or equivalent. No experience required.
Equipment:
Standard office equipment.
Working Environment:
Standard office and hospital environments.
