JOB SUMMARY:
To manage and facilitate the coordination of the Care Management Department.
MAJOR DUTIES / CRITICAL TASKS:
- Collaborates with
nursing and physicians to improve services to meet patient’s psychosocial and
discharge/follow up needs during their medical treatment
- Coordinates the hiring
of new personnel and ensures that they are trained appropriately
- Monitors and reports the
outcomes of consult requests
- Anticipates problems and
plans strategies for the problems/issues
- Develops and maintains
department policies and procedures
- Provides leadership for
the department and the Team Leaders
- Coordinates the
departments quality improvement program
- Ensures that the
discretionary budgets are developed and maintained
- Monitors and reports all
data entries and identifies appropriate internal controls for the department.
Provides mechanisms to monitor and enforce compliance
- Adheres to internal
controls established for the department
- Performs related duties

REQUIRED EDUCATION / EXPERIENCE:

Masters degree in Social Work and six years
experience, two of which must be supervisory experience. Eligible for Texas
Social Work licensure at the LCSW level. Licensure must be obtained within
one year of employment.
OR
-
Registered Nurse with six years experience,
two years which must be supervisory and 2 years which must be in case
management.
EQUIPMENT:
PC based operations, telephone use, copier, charts, pneumatic tube system and
card embossers
WORKING ENVIRONMENT:
Intense high activity work area; requires multiple task management. Requires
flexibility with assigned tasks, as they change to meet departmental needs.
