Job Summary:

To provide social service assistance to assigned population.

Major Duties / Critical Tasks:

• Assists patients and families in accessing community resources in their area.
• Provides social evaluations/ assessments to families to determine needs and appropriate services.
• Provides emotional support to patients and their families.
• Attends educational seminars to keep abreast of current knowledge in the field of social work.
• Provides counseling to patients and their families.
• Adheres to internal controls established for department.
• Participates in discharge planning.

Required Education / Experience:

Master's degree in related field and no experience required. Must be Licensed Master Social Worker.

Standard office equipment.
