JOB SUMMARY: 

To meet all the social service needs of patients in an assigned area. 

MAJOR DUTIES / CRITICAL TASKS: 

• Provides counseling and emotional support to patients and their families.

• Conducts assessment of needs to determine patient's needs and ensure a safe discharge.

• Refers patients to appropriate agencies.

• Adheres to internal controls established for department.

• Coordinates services between physicians, outside agencies, etc.

REQUIRED EDUCATION / EXPERIENCE: 

Bachelor's degree in related field and no experience required.  Must have LBSW (Licensed Baccalaureate Social Worker).   

EQUIPMENT: 

Standard office equipment. 

WORKING ENVIRONMENT: 

Standard hospital or clinic environments.
