JOB SUMMARY: 

To provide comprehensive resource and referral assistance to patients in an assigned area. 

MAJOR DUTIES / CRITICAL TASKS: 

• Conducts interviews with patients and their families to determine needs.

• Provides referrals to outside agencies as appropriate.

• Networks with community resource agencies.

• Provides emotional support to patients and their families.

• Educates staff and patients on various resources available in Texas.

• Prepares documentation as needed for each case.

• Adheres to internal controls established for department.

• Performs related duties as required.

REQUIRED EDUCATION / EXPERIENCE: 

Bachelor's degree in related field and no experience required.  Licensed Mental Health Professional in the State of Texas. 

EQUIPMENT: 

Standard office equipment. 

WORKING ENVIRONMENT: 

Standard hospital, clinical and/or office environment.
