Changing Your Benefits
There are only two occasions that allow an employee to change his or benefits during the plan year – during annual enrollment and a qualifying change of status.
You have 31 days from the date of the change in status event to notify your institution and submit your elections. If you do
not make your changes during the 31-day period, your changes cannot be made until the next annual enrollment in July,
to be effective the following September 1st.
to complete and print the Benefits Enrollment form for your change in status.
Marriage Divorce and Declaration of Informal Marriage (Common Law) – with a copy of a court issued document (State of Texas recognized marriage license, signed final divorce decree or declaration of
Birth, adoption, legal guardianship, disqualification or requalification of a Dependent Qualified Medical Support Order – with a copy of a court issued document, certification of birth, adoption determination,
documentation support dependent eligibility or ineligibility or State issued medical support order.
If you are adding a dependent – a biological child, a grandchild, stepchild or adopted child – please complete the Dependent Application in addition to the required change form.
Involuntary loss of another group coverage, loss or gain of spousal employment and death of a covered member or dependent – with a copy of an original death certificate or documentation supporting the loss
or gain of coverage and/or employment.
An employee whose dependent loses insurance coverage under the Medicaid or CHIP program as a result of loss of eligibility of either the employee or the dependent;
or whose dependent becomes eligible for a premium assistance subsidy under Medicaid or CHIP may enroll this dependent in the basic coverage under UT Benefits, as long as the dependent meets all other UT
eligibility requirements and is enrolled within 60 days from the date of the applicable event. If enrollment of the dependent is conditioned on enrollment of the employee, the employee will also be eligible to enroll.
