Service Request Form

The purpose of this form is to make it easier for you to request our services in a way that will streamline the process and be interactive. Through this process, TOD has a systematic way of recording requests.

Upon receipt of your request, you will receive an email within five business days that confirms receipt with an estimated time of when to receive a response. If you haven’t gotten a response by that time, please feel free to follow up with us by contacting Dr. Marlon Sukal at masukal@UTMB.edu.
